
In my own experience, I’ve seen firsthand how mattress choice can make a significant impact on spinal health and overall pain levels. A few years ago, my wife developed chronic morning low back pain that wouldn’t respond to typical treatments. She tried everything–stretching, massage, adjustments–but nothing seemed to work.
In an effort to find a solution, I suggested she try sleeping on the floor. It might sound extreme, but when I looked into global sleep habits, I found that sleeping on firmer surfaces is common in many cultures. To our surprise, after switching to the floor, she woke up the next morning without any back pain. She continued alternating between the bed and the floor, and each time, she found her pain was worse when she slept on the bed. After several trials, she made the switch to sleeping on the floor permanently. I wasn’t thrilled with this idea for myself, but I joined her out of solidarity, and while the first few nights were uncomfortable, I eventually found it surprisingly comfortable.
Two years later, we both still sleep on the floor, and I began to think: Could this change be contributing to my improved bone density? I’ve noticed a significant improvement in my bone density since making the switch, and I attribute it to the increased mechanical strain on my body from the firmer surface. Could this also explain why sleeping on the floor might help reduce back pain by making our bodies more resilient and less fragile?
Additionally, I hypothesize that one of the key benefits of sleeping on a firm surface, like the floor, is that it encourages more frequent repositioning during sleep. When I used to sleep predominantly on my right side from 2014-2018, I developed a shoulder issue that eventually led to a dislocated shoulder. The MRI showed a supraspinatus and SLAP tear. I now believe that staying in one position on a soft mattress contributed to this injury. In fact, if you want to experience pain, force yourself to remain in one position without moving for 8 hours. By sleeping on a firm surface, the discomfort caused by staying in one position forced me to change positions more frequently, which likely helped reduce strain on my muscles and joints. This increased movement may prevent injury and help alleviate chronic pain—something I’ve experienced personally.
The Historical and Global Context: Soft Mattresses Are a Recent Luxury
Interestingly, what we consider “normal” today–sleeping on soft, cushioned mattresses–is a relatively recent development in human history. For most of our existence, humans slept on hard surfaces, either directly on the ground or on simple bedding like straw, furs, or woven mats. The idea of “comfortable” sleep on plush, soft mattresses is only a modern luxury. Historically:
- Ancient Civilizations: In ancient Egypt, Greece, and Rome, sleeping surfaces were typically raised platforms with minimal bedding–often just blankets, furs, or simple mats. The idea of a “comfortable bed” was rare, and people slept in whatever arrangement they could afford.
- Middle Ages: In Europe during the medieval period, the wealthy used mattresses stuffed with feathers or wool, but these were still firm compared to today’s standards. Common folk slept on straw mattresses or directly on the ground.
- Industrial Revolution (1800s): The rise of industrialization in Europe and America led to the development of the first spring mattresses, which were firmer and more supportive than the soft bedding that followed in the 20th century.
Globally, many cultures still maintain the practice of sleeping on firmer surfaces. For example:
- In Japan and Korea, traditional futons are placed directly on tatami mats, which provide firm support, and people sleep on the floor or low platform beds. The firm surface encourages more frequent repositioning and may contribute to better spinal alignment and lower back health.
- In India and parts of Africa, many people sleep on mats or woven beds, avoiding the overly soft mattresses that are common in the West. These firmer surfaces are consistent with the traditional practice of sleeping on the ground.
- In the Middle East and parts of Asia, sleeping on hard, low beds or directly on the floor is still common.
It’s only in the past century, with the rise of Western consumerism and technological advancements, that mattresses have become significantly softer, with memory foam and luxury foam layers designed to promote comfort. Yet, many of the modern-day back pain issues that people face might stem from a lack of movement during sleep and the excessive softness of mattresses that allow the body to remain in one position for too long.
There is evidence suggesting a correlation between mattress firmness and the incidence of low back pain (LBP) across different nations. For example, a comparison of workers’ compensation claims for back pain between Washington State and Japan revealed that the claim rate in Washington State was 60 times higher than in Japan. 39.0% of U.S. adults reported experiencing back pain in the past 3 months, and a 2023 analysis indicated that 8.2% of American adults suffer from chronic severe back pain. Comparatively, a 2022 survey reported that the overall prevalence of LBP in Japan was 102.1 per 1,000 people or 1%. Yet Japanese Americans report similar back pain incidence to all Americans in general.
Chemical Concerns in Modern Mattresses
While we all want a good night’s sleep, there’s a growing concern about the chemicals used in many modern mattresses. These chemicals, often added to improve fire resistance, durability, or softness, can pose potential risks to our health. From flame retardants to volatile organic compounds (VOCs), mattresses can be a source of chemical exposure that many consumers overlook.
Common offenders include:
- Flame Retardants (PBDEs, TDCIPP): Often used to meet fire safety standards, these chemicals are linked to endocrine disruption, cancer, and neurotoxicity. Even though many PBDEs are now banned, older mattresses may still contain them. A study published in Environmental Health Perspectives in 2008 found that PBDE levels were higher in individuals who slept on foam products.
- VOCs (Formaldehyde, Toluene, Xylene): These volatile chemicals are used in the production of foam and adhesives. They can irritate the skin, eyes, and respiratory system and have been linked to long-term health effects like cancer.
- Perfluorinated Compounds (PFCs): Used to make mattresses more water-resistant, these “forever chemicals” can accumulate in the body and have been associated with developmental issues, liver damage, and immune dysfunction.
- Phthalates: Often found in waterproof covers, phthalates are endocrine disruptors that can impact reproductive health.
If you’re concerned about chemical exposure, the best way to steer clear is to look for mattresses made with natural, organic materials that are free from toxic chemicals. Certifications like CertiPUR-US (for foam materials) or Global Organic Textile Standard (GOTS) can help identify mattresses that meet non-toxic standards. Opting for organic mattresses made from latex, wool, and cotton can also minimize exposure to harmful chemicals.
Protecting Yourself from Mattress Industry Practices
The mattress industry often employs confusing marketing tactics and questionable sales practices that make it difficult for consumers to make informed decisions. Here are a few things to watch out for:
- Confusing Marketing and Rebranding: Many mattresses, which are the same, are sold under different names at different retailers, making price comparisons nearly impossible.
- High-Pressure Sales Tactics: Commission-based salespeople may push you toward more expensive models. Be cautious of upselling and limited-time promotions that encourage impulse buying.
- “Exclusive” Mattress Models: Mattress brands often create “exclusive” versions of their models for specific retailers, which may be nearly identical to other models but sold at a higher price.
To protect yourself, do your research before making a purchase. Look for transparent brands that clearly list the materials and construction of their mattresses. Always check the fine print of return policies, and take advantage of sleep trials—but be cautious of return fees and complicated processes.
The Dearth of Quality Research on Mattresses
Despite the mattress industry’s heavy marketing claims, there is a significant lack of robust, high-quality research on the direct effects of mattresses on sleep quality, pain reduction, and spinal alignment. I could only find one quality study, and it pointed to the fact that firmness plays an important role in providing support for the spine and reducing pain.
Spinal Alignment: A medium-firm mattress is generally the most effective at maintaining proper spinal alignment. Studies, including one in The Lancet (2003), found that medium-firm mattresses significantly reduced chronic low back pain.
While more research is needed, the existing evidence suggests that a medium-firm mattress offers the best balance between comfort and support, especially for those dealing with back pain.
The Takeaway: Mattress Shopping with Confidence
Choosing the right mattress is more than just about comfort—it directly affects your spinal health, sleep quality, and overall well-being. But with chemical exposure, questionable industry practices, and a lack of solid research, mattress shopping can be overwhelming. Here’s how you can protect yourself:
- Look for non-toxic materials: Choose mattresses made from organic, chemical-free materials, and look for certifications like CertiPUR-US or GOTS.
- Be cautious of high-pressure sales tactics: Take your time to compare prices and make sure you’re getting a fair deal. Always read the fine print of return policies.
- Opt for medium-firm: Research suggests that a medium-firm mattress provides the best support for spinal alignment and pain relief.
Or you can bypass all these hassles and expenses and get a Japanese tatami or sleep on the floor. I personally sleep on a thin mattress topper or pad which I place on the floor. Granted, sleeping on such firm surfaces is not ideal for everyone. Decades of sleeping in plushness may make such a transition intolerable in which case go for ‘medium firm’.
